Pharmacy Student Profile
If you wish to submit this form by e-mail, you must save the form to your home share or hard drive before filling out any information. The form can then be sent as an e-mail attachment. The Student Profile is sent to your preceptor prior to your arrival at the site. It is the first impression you make on your preceptor. Therefore, be sure the Profile is filled out with correct spelling, grammar, and punctuation. Do not use the Enter key in the Profile document. Use the Tab key to move to the next gray block. Press Control + Tab to tab five spaces on the same line or Shift + Tab to go back a block. Save the profile to your home share or hard drive for future use. Send the profile as an attachment to karen.franceschina@umontana.edu or print a hard copy and take it to SB 323.

IPPE and APPE Site Preference Forms
Review the community or hospital pharmacy Introductory Pharmacy Practice Experience (IPPE) site list for available sites and select two sites you would be interested in. If you wish to submit this form by e-mail, you must save the form to your home share or hard drive before filling out any information. The form can then be sent as an e-mail attachment. Do not use the Enter key in the Preference Form documents. Use the Tab key to move to the next gray block. Press Control + Tab to tab five spaces on the same line or Shift + Tab to move back a block. After filling in the necessary information on the selected form, send it as an attachment to karen.franceschina@umontana.edu or print a hard copy and take it to SB 323. If you are completing the Internship for Introductory Pharmacy Practice Experience (IPPE) Preference Form, this means you have located your own site. Be sure to fill it in completely and accurately so the information can be confirmed in a timely manner.
